Electronic Replacement Certificate Request

Important update

In response to the initial remote working requirements during the COVID-19 pandemic, Active IQ began issuing certificates via an electronic eCertificate only.

Based on positive feedback and the success of eCertificates as an effective digital solution for our customers, eCertification will be the default certification format throughout the remainder of the COVID pandemic and also once office premises reopen.

How to request an electronic replacement certificate

To request an electronic replacement certificate please download and complete this form.

Each electronic replacement certificate costs £15 plus VAT. Please send the completed form to Active IQ, Dryden House, St. Johns Street, Huntingdon, Cambridgeshire, PE29 3NU, or submit an authentically signed electronic copy to

Payment can be made by card. Please allow for Active IQ to complete all relevant checks and our Accounts team will then contact you on the telephone number you provide on the replacement certificate form.

  • Electronic replacement certificates are issued within five working days.
  • Active IQ cannot issue certificates until payment has cleared.
  • We cannot enter in to any correspondence regarding awards, which do not appear on our records.

Please note that only if the reason for the replacement is acceptable to Active IQ will an electronic replacement certificate be issued (e.g. lost, stolen or destroyed). We also reserve the right to contact the Training Provider that you achieved your qualification with.
